*Medication adherence issue is defined as a medication adherence score (MAS) of less than 80%. The MAS is determined by comparing how a patient is supposed to be taking a medication based on the active medication list in EPIC with the amount of medication that has been dispensed. For example, if a patient is expected to have used 360 metformin pills in the last 6 months, but has only been dispensed enough for 180 days, the MAS is 50%.
Wizard Tools are not available for patients over age 75 because care for this age group requires a greater degree of clinical judgment and individualization. Wizard Tools online can only be used for patients age 18-75. If the rooming nurse did not get a BPA it might mean:
The clinical issues are listed in order of priority, with the ones giving greatest benefit at the top. Keep in mind, the conditions listed on the patient version with check marks and “keep up the good work” messages are meant only for positive reinforcement to patients and do not need to be addressed at the visit.
The score was removed from the patient version because patients told us in focus groups and through MY VOICE that they did not understand the score and it frightened some of them. The risk score is still available on the “provider version” which can be shared with appropriate patients at the discretion of the provider along with an opportunity for questions to be answered.
We have already great feedback and many great suggestions through the “suggestions” tab that providers access through Wizard Tools within the visit navigator. We are also actively conducting provider interviews. All feedback we receive is being tracked and analyzed. Changes to Priority Wizard to incorporate learnings will be released on June 19. In the meantime, keep your comments and suggestions coming!
